Access Community Health

Our team of registered health professionals and trained support workers will work in partnership with you and your family to provide a flexible service that is tailored to your needs and promotes rehabilitation, enabling you to return to as much independence as possible.

Disability support services are provided for people of all ages who have a physical or intellectual disability and need assistance with their activities of daily living. We do a needs assessment to determine whether we can help you or your family.

Access Injury Support services range from a helping hand around the house to support with daily living activities, rehabilitation and specialist nursing services. As one of the lead providers, we are committed to providing excellence in specialised injury related support to assist your recovery and return to independence.

Home and community support includes a range of services available for older people who need support to live at home. 

Our high quality support, provided by our team of registered health professionals and trained support workers, is tailored to meet your needs, rather than a blanket approach and helps you to live as independently as possible. You will be supported to remain at home safely, while giving family members peace of mind knowing you are receiving the best support possible.

Our palliative care team works together with your health care team to:

• provide personalised, holistic care
• make you as comfortable as possible
• enable you to spend time with the people closest to you
• minimise the physical and emotional stress that you, your family and loved ones may experience
• ensure that your rights are respected and appropriate care is accessible
• build community systems to support you and your family

Individualised Funding is a Ministry of Health funded support service that gives eligible people the flexibility and control to manage their own funding, in a way that helps them live their life in a way that best suits you.
