Community Health Hubs

Community Health Hubs

Community Health Hubs are community-focussed and co-located group of health providers. They will be develop in advance of and to complement the new Dunedin Hospital.

In addition to housing at least one Health Care Home family practice, services potentially to be delivered at Community Health Hubs may include many outpatients clinics, day surgery, pre-admission clinics, primary birthing, rehabilitation services, telehealth clinics, expanded mental health services, renal dialysis, nurse led and allied health led clinics. There may also be community based providers such as a pharmacy, physiotherapy or audiology services, for example. 

The scale and services of CHH will be determined by the population size, health needs, demographics and existing infrastructure and Locality Networks will help to determine this

All providers in a community hub are committed to delivering services that are well-integrated, including with other parts of the Southern health system.

Currently a number of the rural hospital trusts – Gore, Balclutha Dunstan and Oamaru - have configured their ambulatory services on a model similar to a community hub.
